After selecting your schedule(s), click the Applying for scholarship/Promissory note button and select what type of scholarship are you applying. Due to Pandemic face to face transaction is limited, there will be additional instructions where you can send email for your scholarship application.
Make sure you process your scholarship application before payment.


FOR FRESHMEN


Download the Medical and Dental Form 2022 and X-ray referral slip at (https://www.umak.edu.ph/student) , fill out the form and have your X-ray. Submit all completed requirements at the Medical and Dental Clinic(Lobby, Bldg 1) to get your clearance.


CHANGE OF RESIDENCY (RVF FORM)


GUIDELINES FOR RESIDENCY VERIFICATION

Residency verification is the one of the steps upon admission and enrollment in the University. This is used in order to classify a student-applicant or student-enrollee as Makati resident or not. In most cases, the University prioritizes Makati residents during admission. Residency helps determine the amount of token fee to be paid during enrollment. Thus, a Residency Verification Form (RVF) from the University of Makati Accounting Office shall be accomplished by the student upon registration for the Senior High School Entrance Aptitude Test and College Qualifying Examination and upon enrollment to the University

A student may be classified as MAKATI RESIDENT if ANY of the following requirements is submitted:

  1. FOR INCOMING SENIOR HIGH SCHOOL STUDENT
    • Verification (list) or Certification to be provided by DepEd Makati
    • For incoming students who are 17 years old and below:
    • SK Voter’s Certificate of the student issued after the recent SK elections OR
    • Voter’s Certificate of Parents issued after the recent local or national elections (whichever is the most recent) OR
    • For incoming students who are 18 years old and above:
    • Voter’s Certificate issued after the recent local or national elections (whichever is the most recent)
  2. FOR INCOMING COLLEGE STUDENT
    • Verification (list) or Certification provided by DepEd Makati or Higher School ng UMak (HSU)
    • For incoming students who are 18 years old and above:
    • Voter’s Certificate issued after the recent local or national elections (whichever is the most recent)

Notes:

  • The names included in the list of DepEd Makati or Higher School ng UMak (HSU), which will be provided to the University, shall be considered MAKATI resident” for one academic year as they are encouraged to register as voter.
  • Failure to submit the above requirements shall mean that the student will be classified as Non-Makati resident; and
  • Any misrepresentation shall warrant expulsion
