Verification Process

  • Enter your Student Number in the box.
  • Authorized representative or visitors may type NA
    • If you typed, NA in the Student Number Field.
    You will be required to enter other details such as Last Name, First Name and Middle Name, Email Address, Contact Number.
    If you forgot your Student Number, please input your full name (for married female, use your Maiden Name)

Schedule an Appointment

  • Multiple appointment may be selected.
  • Schedules available are from Mondays to Fridays, 8:30 AM to 4:00 PM except holidays or during declared suspension of work.
  • Select the purpose for the Appointment Schedule.
    Example: Residency Verification Form, Office to visit CASH OFFICE.
