STEPS IN THE MEDICAL EXAM FOR ENROLLMENT:

1. Secure The Notice of Admission from the Admissions Office
2. Download and print the Medical and Dental Form (back to back in long bondpaper) at https://www.umak.edu.ph/student/forms/. Fill out the form and attach a passport size picture.
3. Download and print the Medical Referral Slip at https://www.umak.edu.ph/student/forms/. Present it to any Laboratory and have your Xray and Drug Test done. (Medical Courses/Law Students may have additional lab exams)
4. After completing steps 1-3, Schedule your appointment below using the Registration Number found in your Notice of Admission (must have a valid email to participate). Appointment Slip will be emailed after successful registration.
5. Proceed at the Medical and Dental Clinic on appointed date and bring the following:
   1. Notice of Admission
   2. Medical and Dental Record
   3. Xray and Drug Test Result (for non-medical courses)
   4. Printed or Softcopy of the appointment slip
   5. Ballpen
